The student has admitted and is documented as per student evaluation, that he has exhibited prior deficiencies in time management and substandard preparation for clinic sessions. It has not been documented sufficiently that the student sought specific opportunities through his mentors to more efficiently help correct the cause of these deficiencies, whether a deficit in clinical knowledge, proficiency or otherwise (ADA, 2006)."
